About The Position

TRU Healthcare is seeking a per-diem Registered Nurse Case Manager passionate about home health and interested in growing with the agency. The role is paid per visit, with compensation varying by visit type, and may have the potential to transition to a part-time or full-time salaried position as the agency grows.

The RN Case Manager manages and schedules patient visits, travels within the community, and provides personalized, one-on-one nursing services in patients’ homes. The position works as part of an interdisciplinary team and provides clinical supervision and mentorship to caregivers to support adherence to care plans. The role offers a Monday-through-Friday schedule, with evenings and weekends free, and uses AI-powered documentation tools to help clinicians complete Starts of Care documentation more quickly.

The position requires a valid Minnesota RN license or Minnesota/North Dakota compact license and a desire to provide direct patient care in the home. Benefits listed are for full-time employees.

Requirements

• Hold a valid Minnesota Registered Nurse license or Minnesota/North Dakota compact license.

Responsibilities
