About The Position

Concho Hearts Hospice is looking to expand our team. As Executive Director, you will build your own team and create your own hospice location to serve your community. Concho Hearts Executive Director/Marketer serves as a leader in the overall organization management of the hospice agency. Key responsibilities include developing people and processes, ensuring high-quality care for each patient, financial management, and business development. Concho Hearts Hospice is dedicated to providing compassionate care wherever you call home. Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers, and doctors provides symptom management and patient and family support while coping with an end-of-life illness. We are on this journey with you, and everyone's journey is different. We are locally owned and operated, so you can rest easy that your loved one will receive loving care in a timely manner.

Requirements

Hospice Marketing experience of 2+ years.

Responsibilities

Develops various channels of referrals through the selection, acquisition, retention, and growth of business partnerships with referral sources.
Consistently visits, communicates, and monitors provider, community, customer, payer, and patient perceptions of Concho Hearts Hospice as a high-quality provider of services.
Conduct educational in-services and community outreach to promote our superior hospice care services.
Educate patients and their families about hospice expectations.
Work closely with the clinical operations team to ensure seamless care transition coordination from an acute care setting to a home.
Operate and maintain compliance standards for patient care.
