Hospice Clinical Coordinator (RN)

About The Position
The Clinical Coordinator – Home Health role coordinates day-to-day hospice and/or home-health patient care services, supports effective operations of patient care needs, and collaborates with the clinical team. The Clinical Coordinator leads, guides, and supports staff in delivering highly reliable, evidence-based, patient- and family-centered quality care.
Requirements
- Bachelor’s degree in Nursing or another advanced clinical degree.
- Appropriate clinical license (RN, PT, or OT).
- Valid driver’s license.
- Three years of home health and/or hospice experience.
Responsibilities
- Coordinate day-to-day hospice and/or home-health patient care services.
- Support effective operations of patient care needs.
- Collaborate with the clinical team.
- Lead, guide, and support staff in providing highly reliable, evidence-based, patient- and family-centered quality care.
