About The Position

The Hospice Certified Nursing Assistant (CNA) provides compassionate, hands-on care that supports the dignity, comfort, and quality of life of patients during end-of-life care. Working under the supervision of a registered nurse and as part of the interdisciplinary hospice team, the CNA delivers personal care and assistance with daily living activities, ensures the safe use of medical equipment, accurately documents services provided, and offers emotional support to patients and families. This role requires professionalism, empathy, and a strong commitment to patient-centered care in accordance with agency policies and regulatory standards.

Requirements

  • Current Certified Nurse Aide (CNA) certification.
  • Current Basic Life Support/CPR certification.
  • Current and valid driver’s license, reliable transportation, and current liability auto insurance.
  • Ability to work well with others and take direction from management, as well as take initiative – willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment.
  • Excellent leadership, problem-solving, and communication skills.
  • High attention to detail and accuracy.
  • Demonstrated ability to manage multiple tasks and deadlines in a fast-paced environment.
  • Self-motivated to accomplish identified goals with a strong sense of accountability for results.

Responsibilities

  • Working in collaboration with the supervising RN, to fulfill the defined patient-specific Hospice CNA Plan of Care.
  • Collaborating with the care team as an extension of nursing services to ensure all patient needs are fully addressed.
  • Delivering hands-on personal care and supportive services to assist with safe activities of daily living, in accordance with the established Hospice CNA Plan of Care, and in compliance with the applicable laws, regulations, and agency policies.
  • Documenting care provided according to agency policy.
  • Using Durable Medical Equipment safely.
  • Participating in activities that promote patient safety, quality, and regulatory compliance.
  • Participating in professional development and lifelong learning activities.
  • Performing other responsibilities as assigned
