About The Position

Moments Hospice is hiring a full-time, field-based Hospice Certified Nursing Assistant (CNA), also called a Hospice Aide. The CNA provides hands-on personal care to hospice patients in their homes, assisted living settings, or care facilities, supporting patients’ comfort and dignity and offering support to their families. The CNA works as part of an interdisciplinary care team and closely with a dedicated RN Case Manager, following each patient’s plan of care under the RN’s direction. Structured training and opportunities to build skills are offered. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. Pay is $18–$23 per hour. A fleet car is provided for business and personal use, with fuel and insurance covered at minimal cost to the employee.

Requirements

• Active Certified Nursing Assistant (CNA) certification.
• Ability to work in an electronic medical record system.
• Strong written and verbal communication and active listening skills.
• Excellent customer service, problem-solving, and time management skills.
• Compassion, patience, and ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and good understanding of body mechanics.
• Valid driver’s license and current auto insurance.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living or memory care, skilled nursing, a hospital, or hospice.
• Current CPR certification.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
