About The Position

Heritage Hospice is resuming services in the Albuquerque community and is hiring a PRN Hospice CNA to join its interdisciplinary care team. The position provides personal care, comfort, and supportive services to patients and families in home and facility settings. It offers flexible PRN scheduling and the opportunity to work alongside a hospice team while supporting patients and families across the assigned coverage area.

Requirements

Current New Mexico CNA certification.
Reliable transportation and a valid driver’s license.
Ability to work independently in home settings with professionalism and compassion.
Strong communication, time-management, and interpersonal skills.
Comfort supporting patients and families during end-of-life transitions.

Nice To Haves

At least six months of direct patient care experience is preferred.
Hospice or home health experience is preferred.

Responsibilities

Provide hands-on personal care, including bathing, grooming, dressing, repositioning, and activities of daily living support.
Help maintain a safe, clean, and comfortable environment in patients’ homes or facility settings.
Observe and report changes in patients’ conditions to the RN Case Manager.
Offer emotional support to patients and families with sensitivity and respect.
Document care accurately in accordance with regulatory and organizational standards.
Communicate with the team to support coordinated, high-quality care.
Travel to patient homes, facilities, and communities within the assigned coverage area.
