About The Position

The Care Team Hospice is seeking a Certified Nursing Assistant to provide personal care and comfort-focused support to hospice patients in their homes and facility settings. The CNA works alongside an interdisciplinary team to support patients’ dignity, comfort, and quality of life and to support their families throughout the hospice journey. The role covers an area within up to a one-hour radius of Mishawaka, Indiana. Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM. Compensation is $18.00–$22.00 per hour, plus mileage reimbursement.

Requirements

• Active CNA certification.
• Active driver’s license.
• Ability to pass a criminal background check.
• Ability to pass a Motor Vehicle Record (MVR) check.
• Current TB testing in accordance with agency policy.
• Current CPR certification as required for the role.

Nice To Haves

• Hospice aide experience.
• Experience using HCHB (Homecare Homebase).

Responsibilities

• Provide personal care, hygiene, and comfort measures.
• Observe and report changes in patient condition.
• Support patients and families with compassion and respect.
• Document care provided accurately and promptly in the EMR, HCHB (Homecare Homebase).
