About The Position

AT Home Care Hospice is seeking a dedicated Hospice CNA to provide compassionate, dignified care and support to patients and families during hospice care. The role provides one-to-one care in patients’ homes across Fredericksburg, Bowling Green, Stafford, Caroline, Spotsylvania, King George, Culpeper, Orange, and Colonial Beach. CNAs work independently with support from the hospice team and collaborate with the interdisciplinary team. The schedule is full-time, Monday through Friday, 8 a.m. to 5 p.m. The stated pay range is $20.00–$23.00 per hour.

AT Home Care Hospice provides hospice services tailored to the individual needs of patients and families, with the aim of delivering care in a home environment. The organization is an affiliate of Adoration Health.

Requirements

• Meet the training requirements of the state of practice.
• Have at least six months of experience, including experience providing personal care.
• Be at least 18 years of age.
• Complete at least 75 hours of classroom and supervised practical training; 16 classroom hours must be completed before the required 16 hours of practical training.
• Possess and maintain current CPR certification.
• Be able to read and follow written instructions and document care provided.
• Understand hospice philosophy and be comfortable providing specialized care to terminally ill patients.
• Be able to work with little direct supervision.
• Be able to handle death and dying.
• Have strong oral and written communication skills.
• Have good organizational skills.

Responsibilities

• Help patients with bathing, dressing, toileting, mobility, and repositioning.
• Observe patients and report changes in their condition.
• Assist with ambulation, transfers, range-of-motion exercises, and safe equipment use.
• Assist with self-administered medications in accordance with state regulations.
• Offer companionship and emotional comfort to patients and families.
• Change linens, assist with meals, and help maintain a clean space.
• Maintain accurate and timely documentation.
• Collaborate with the hospice interdisciplinary team.
