About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Livonia, MI area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The CNA will provide hands-on care, assist with activities of daily living (ADLs), observe and report changes in patient condition, and support patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand the hospice journey. Responsibilities include responding promptly to patient needs with empathy and attentiveness, accurately documenting care in the EMR, and traveling to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Valid driver’s license
• Ability to spend approximately 20% of your workday driving to and from patient locations

Responsibilities

• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
