About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The Hospice Aide - CNA provides direct patient care under the direction of an RN and in accordance with the Hospice Aide/Homemaker Plan of Care. The role also involves supporting safe transfers and ambulation, communicating and documenting changes and care, following infection-control principles, and promoting respectful communication with patients, families, and coworkers.

Requirements

High school diploma preferred or GED.
Unencumbered certification according to the applicable Nurse Aide Registry.
Current CNA license.
At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
Demonstrated interest in the welfare of ill and elderly people and a commitment to compassionate care.
High integrity, including maintenance of confidential information.
Current driver's license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

EMR experience is a plus.

Responsibilities

Provide direct patient care under the direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
Assist patients with safe transfers and ambulation according to agency policy.
Report and communicate changes, and document pertinent information and care provided to support continuity of care.
Practice accepted infection-control principles.
Promote positive, supportive, and respectful communication with patients, families, and other employees.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
