About The Position

Phoenix Home Care & Hospice is seeking a Home Health RN to provide care to patients in Kansas City, Missouri, and surrounding communities. This full-time role follows a 7-days-on, 7-days-off schedule and includes on-call overnights and weekends. The salary range is $80,000–$95,000.

The nurse provides skilled visits tailored to patient needs, supports patients through daily interaction, and delivers clinical care and education in the home. The role includes wound care, medication management, patient-condition documentation, and collaboration with other healthcare professionals. Requirements

• Active RN nursing license in the state.

Responsibilities

• Provide skilled visits to patients based on their needs.
• Interact with patients daily to ensure their comfort and support.
• Educate patients about health conditions, including CHF, COPD, and diabetes.
• Perform wound care and assessments.
• Manage and administer medications.
• Change dressings, bandages, and other prescribed devices or contraptions.
• Document patient conditions and report significant changes to the physician.
• Collaborate with healthcare professionals, including physical, speech, and occupational therapists.
