About The Position

Elara Caring is hiring a full-time Home Health RN Case Manager serving the Northampton-Easthampton territory. The RN provides skilled nursing care in patients’ homes, helping them recover, manage their health, and remain independent. The role guides patients through the home health care process from assessment to discharge, with flexibility in planning the workday and one-on-one patient visits. Compensation is a guaranteed salary of $87,000 to $97,000, based on experience, plus a productivity bonus.

Requirements

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to work independently and travel throughout the assigned territory.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 pounds.

Responsibilities

• Provide skilled nursing visits in patients’ homes.
• Guide patients through home health care from assessment to discharge.
• Develop and update patient care plans.
• Manage an assigned caseload.
• Coordinate care with physicians and other clinicians.
• Teach patients and caregivers to manage medications, treatments, and health conditions at home.
