About The Position
AHN Healthcare@Home is hiring a Registered Nurse Case Manager for a home health position providing skilled nursing care to patients in their homes in the Robinson, Pennsylvania area. The role offers autonomy, meaningful patient relationships, and career growth while working with a supportive interdisciplinary team. The position includes up to $25,000 in total bonus potential over the first three years, subject to good standing, performance expectations, and remaining in the role and status for which the employee was hired. Bonus opportunities include a $10,000 first-year success bonus, up to $500 per month in Year 2, and up to $750 per month in Year 3. Monthly bonus eligibility may grow to $1,000 after four years. The nurse works independently in the field, traveling to patients’ homes within an assigned territory. A dedicated field education team provides support from the first day, alongside leaders who mentor employees and a collaborative culture that recognizes impact and incorporates feedback.
Requirements
- Active Registered Nurse (RN) license in the state where the employee works.
- One year of RN experience and the clinical expertise to deliver quality patient care.
- Valid driver’s license.
- Ability to spend approximately 20% of the workday driving to and from patient locations.
Responsibilities
- Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
- Perform patient assessments, medication management, and care planning.
- Educate and empower patients and families about disease management and treatment plans.
- Think critically and solve problems in real time.
- Complete thorough, compliant EMR documentation.
- Travel to patient homes within the assigned territory.
