About The Position

Elara Caring is hiring a full-time Home Health RN Case Manager serving the Northampton-Easthampton territory. The nurse provides skilled care in patients’ homes, helping them recover, manage their health, and maintain independence. The role offers a guaranteed salary of $87,000 to $97,000 based on experience, plus a productivity bonus. The work includes one-on-one patient visits, with flexibility in planning the workday and support from clinical leaders and an interdisciplinary team.

Requirements

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to work independently and travel throughout the assigned territory.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 pounds.

Responsibilities

• Guide patients through home health care from assessment to discharge.
• Provide skilled nursing visits in patients’ homes.
• Develop and update patient care plans.
• Manage an assigned caseload.
• Coordinate care with physicians and other clinicians.
• Teach patients and caregivers to manage medications, treatments, and health conditions at home.
