About The Position
AHN Healthcare@Home is hiring a Registered Nurse Case Manager for a home health position providing skilled nursing care to patients in their homes in the Rostraver, Pennsylvania area. The role offers autonomy, meaningful patient relationships, field education beginning on Day 1, and support from an interdisciplinary team and leadership. The position offers up to $25,000 in total bonus potential over the first three years, subject to good standing, performance expectations, and remaining in the hired role and status. Bonus opportunities include a $10,000 first-year success bonus, up to $500 per month in the second year, and up to $750 per month in the third year; monthly bonus eligibility may grow to $1,000 after four years. The position is field-based and requires travel to patient homes within the assigned territory. Employees spend approximately 20% of their workday driving between patient locations. The job is 40 hours per week.
Requirements
- Hold an active Registered Nurse (RN) license in the state where the employee works.
- Have at least one year of RN experience and the clinical expertise to deliver quality patient care.
- Have a valid driver’s license.
- Be able to spend approximately 20% of the workday driving to and from patient locations.
- Successfully complete a pre-employment drug screen as part of the background process.
- Be legally authorized to work in the United States.
Responsibilities
- Provide skilled, patient-centered nursing care to patients in a home health setting in collaboration with an interdisciplinary team.
- Perform patient assessments, medication management, and care planning.
- Educate and empower patients and families about disease management and treatment plans.
- Apply critical thinking and solve problems in real time.
- Complete thorough, compliant electronic medical record documentation.
- Travel to patient homes within the assigned territory.
