About The Position
The Registered Nurse (RN) Home Health performs patient visits and coordinates client care. The RN serves as the point of contact for all disciplines involved in patient care and oversees visit frequency for each episode. The role consults with physicians and the office as needed to provide information about patient care. Key duties include accurate OASIS collection, maintaining current medication profiles, ensuring lab values are reported to the physician promptly, attending weekly case conferences, and participating in monthly case manager meetings.
Requirements
- Currently licensed as a Registered Nurse (RN) in the state of employment.
- Demonstrate knowledge and skill in current nursing practice.
- Possess a valid state driver’s license and automobile liability insurance.

- A minimum of one year of clinical experience is preferred.
Responsibilities
