About The Position

This is a PRN-only RN Case Manager (RNCM) and Recertification Nurse position. The role involves consistently accepting and executing RNCM and recertification visits, weekly medicine setups, and providing in-office State Survey support as needed. There is potential for growth into a Part-Time or Full-Time RNCM position. Contractors will also be considered. This role requires a mix of RNCM services and recertification visits, including travel, across the far-West Northwestern Ohio region. The position works closely with and reports directly to the NW OH Regional Program Administrator and Director of Clinical & Administrative Operations. There is a defined path for advancement to various leadership roles within the agency. Work includes case management, starts of care, admissions, recertifications, transitional care, resumptions of care, clinical reviews, assessments, new hire orientations, in-service and supervisory reviews, and input to influence training, care practices, and business success. The agency is nurse-owned and locally led, prioritizing patient care and well-being. This PRN position offers a long-term employment opportunity that can be built to fit a schedule, with no overnight shifts or overnight travel required. Mileage reimbursement is provided.

Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of one (1) year of experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience.
  • 1+ years of paid home health / in-home care experience is also required.
  • Minimum of one (1) year Experience in case management is required.
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date).
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • At least 1 year of experience in Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience at/with a home health agency or in-home care provider (or equivalent).

Responsibilities

  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document/Communicate/Coordinate patient status/change in condition to the care team.
  • Promote personal safety and a safe environment for clients and coworkers.
  • Travel to surrounding counties.
  • Coordinate and provide patient care and ensure optimal outcomes.
  • Oversee patient services and provide expert nursing care.
  • Handle complex cases utilizing critical care and infectious disease care skills.
  • Lead and support a multidisciplinary team.
  • Ensure successful communication with physicians, agency staff, service administrators, other caregivers and clinicians, and the client and their family.
