About The Position

Pediatric Nursing Specialists of Indiana (PNS), a Home Care Network (HCN) agency, is seeking an experienced Home Health RN Case Manager (RNCM) and Recertification Nurse for a PRN or Contractor role. This position offers a flexible schedule, typically 1-15 daytime hours per week, Monday-Friday, 8:00 AM to 5:00 PM, with occasional early evening and rare weekend visits. The role involves case management, starts of care, admissions, recertifications, transitional care, resumptions of care, clinical reviews, and other assessments and services. It also includes new hire orientations, in-service and supervisory reviews, and contributing to training, education, and care practices. Travel throughout Indiana, within a 1-hour radius of Indianapolis (up to 2 hours occasionally), is required. The agency is nurse-owned and locally led, focusing on patient care and well-being. There is potential for growth into Part-Time or Full-Time positions and other leadership roles within the agency.

Requirements

  • Graduate of an accredited School of Nursing.
  • Active RN License / ability to practice in the State of Indiana.
  • Minimum of one (1) year of nursing supervisory experience.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience.
  • 1+ years of paid home health / in-home care experience is also required.
  • Minimum of one (1) year experience in case management is required.
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date).
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • Willingness to consent to a background check, including a criminal record check, and an employment and education verification.

  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.

Responsibilities

  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document/Communicate/Coordinate patient status/change in condition to the care team.
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Travel to surrounding counties.
  • Provide case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services.
  • Conduct new hire orientations, in-service and supervisory reviews.
  • Provide input to influence training and education, care practices and outcomes, and the success of clients, employees, and the business.
  • Coordinate and provide patient care and ensure optimal outcomes.
  • Oversee patient services and provide expert nursing care.
  • Ensure successful communication with and between physicians, agency staff, service administrators, other caregivers and clinicians, and the client and their family.
