About The Position

Pediatric Nursing Specialists of Indiana (PNS), a Home Care Network (HCN) agency, is seeking an experienced RN Case Manager to join their team as a PRN or Contractor. This leadership position involves case management, recertification visits, and potential support for State Surveys in the Indianapolis office. The role offers a flexible schedule, typically 1-15 daytime hours per week, Monday-Friday, with potential for growth into part-time or full-time positions. Requirements

Graduate of an accredited School of Nursing.
Minimum of one (1) year of experience as a Registered Nurse, with an active RN License / ability to practice in the State of Indiana.
Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience.
1+ years of paid home health / in-home care experience is also required.
Minimum of one (1) year Experience in case management is required.
CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
Current Driver's License and reliable transportation.
Willing to travel surrounding counties.
At least 1 year of experience in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent).

2+ years of both home health and acute care experience are preferred.
Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
Familiarity with HIPAA regulations and medical terminology.

Responsibilities

Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
Document/Communicate/Coordinate patient status/change in condition to the care team.
Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
Travel to surrounding counties.
Provide case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services.
Conduct new hire orientations, in-service and supervisory reviews.
Provide input to influence training and education, care practices and outcomes, and the success of clients, employees, and the business.
