About The Position

Pediatric Nursing Specialists of Indiana (PNS), a Home Care Network (HCN) agency, is seeking an experienced RN Case Manager to join their team as a PRN or Contractor. This leadership position involves case management, recertification visits, and potential support for State Surveys in the Indianapolis office. The role offers a flexible schedule, typically 1-15 daytime hours per week, Monday-Friday, with potential for growth into part-time or full-time positions. Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of one (1) year of experience as a Registered Nurse, with an active RN License / ability to practice in the State of Indiana.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience.
  • 1+ years of paid home health / in-home care experience is also required.
  • Minimum of one (1) year Experience in case management is required.
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
  • Current Driver's License and reliable transportation.
  • Willing to travel surrounding counties.
  • At least 1 year of experience in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent).

  • 2+ years of both home health and acute care experience are preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Familiarity with HIPAA regulations and medical terminology.

Responsibilities

  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document/Communicate/Coordinate patient status/change in condition to the care team.
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Travel to surrounding counties.
  • Provide case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services.
  • Conduct new hire orientations, in-service and supervisory reviews.
  • Provide input to influence training and education, care practices and outcomes, and the success of clients, employees, and the business.
