About The Position

Home Care Network, Inc. is seeking a skilled and self-motivated Registered Nurse Case Manager and Recertification Nurse to serve home health clients throughout Northwest Ohio. This field staff leadership role involves coordinating and reassessing clients’ ongoing home health needs, confirming medical necessity for continued services, providing nursing care, and supporting optimal patient outcomes. The RN submits timely documentation in compliance with state, federal, and agency guidelines and communicates with physicians, agency staff, administrators, caregivers, clinicians, clients, and families. The position includes case management, starts of care and admissions, recertifications, transitional care and resumptions of care, clinical reviews, assessments, and other services. It also includes new-hire orientations, in-service education, supervisory reviews, and input into training, care practices, and outcomes. The nurse manages or supervises RNs, LPNs, home health aides, and personal care aides, and may provide backup skilled nursing visits and services such as medication sets, dressing changes, PICC/PORT/IV care, and lab draws. Two service-area arrangements are offered. The Regional RNCM option is PRN and involves travel to clients within a 60-minute radius, with occasional travel to Tiffin, Fostoria, or Findlay highly desired. The Road Warrior RNCM option is PRN or part-time and covers Northwest Ohio within a two-hour radius; willingness to travel throughout that region is required, and this option receives first priority for cases. Both options require a recurring weekly schedule, agreed-upon availability, and specified case acceptance and visit minimums. The schedule is primarily weekday daytime, with occasional evening availability; weekends, on-call duty, and overnight travel are not required. The agency describes the role as long-term employment and offers customizable schedules, mileage reimbursement, in-house scheduling and call-center support, quality and compliance support, and training. Compensation for part-time or PRN work is $36–$40 per hour, based on the employment and travel agreement, plus mileage. Overtime and quarterly bonus opportunities are available. Full-time RNCMs may choose hourly or salaried pay and may qualify for PTO and health, dental, and vision coverage.
