About The Position

Home Care Network, Inc. is seeking an experienced, self-motivated Registered Nurse to serve existing and returning home health clients throughout Northwest Ohio in a part-time or PRN field staff leadership role. The position involves case management, starts of care, admissions, recertifications, transitional care, resumptions of care, clinical reviews, assessments, and other nursing services. The nurse also contributes to new-hire orientations, in-service education, supervisory reviews, and the development of training, care practices, and client outcomes. The role includes directing and planning care, supervising a team of RNs, LPNs, Home Health Aides, and Personal Care Aides, coordinating with physicians and agency staff, documenting changes in patient status, and confirming the ongoing medical necessity of home health services. Occasional backup skilled nursing visits may include medication sets, dressing changes, and related care. Updated patient documentation must be submitted promptly and in accordance with applicable state, federal, and agency guidelines. The position reports to the NW OH Regional Program Administrator, or to the Clinical Administrator/Director of Nursing. It is a weekday role with primarily daytime availability, generally 8 a.m. to 5 p.m., and occasional evening availability. Weekend work is not required; optional weekend services may be available. There are no on-call requirements or overnight travel. Travel is required within the agreed service area: up to a 60-minute radius for the regional PRN option, or throughout a two-hour radius of Northwest Ohio for the road-warrior option. The road-warrior option is PRN or part-time and receives first priority for cases. Mileage is reimbursed round-trip/door-to-door. Compensation is $36–$40 per hour, depending on employment and travel agreement and expected availability; overtime and quarterly bonus opportunities are available. Full-time RNCMs are eligible for additional benefits, but this listing is for part-time or PRN employment.
