About The Position

Home Care Network, Inc. is seeking an experienced Registered Nurse to serve as a field staff leader for existing and returning home health clients across Northwest Ohio. This part-time or PRN role focuses on case management, recertifications, patient assessments, and the coordination and delivery of home health services. The nurse works directly with clients and leads care delivered by RNs, LPNs, home health aides, and personal care aides. Duties include starts of care, admissions, recertifications, transitional care, resumptions of care, clinical reviews, other assessments and services, and occasional skilled nursing visits such as medication sets, dressing changes, PICC/PORT/IV care, and lab draws. The role also includes new-hire orientations, in-service education, supervisory reviews, and input on training, care practices, and outcomes. The nurse coordinates with physicians, agency staff, service administrators, caregivers, clinicians, clients, and families, and submits timely documentation supporting patients’ conditions and ongoing care needs in accordance with applicable requirements. The position is scheduled weekdays, primarily during daytime hours, with occasional evening availability. Weekends and overnight travel are not required; weekend services may be optional. Two service-area options are offered: PRN coverage within a 60-minute radius, or PRN/part-time coverage across a broader Northwest Ohio region within a two-hour radius. The broader-region option receives first priority for cases to help maximize hours. Both options require an agreed case-acceptance rate and at least one case worked every 15 days, with exceptions. The role reports to the Regional Program Administrator or Clinical Administrator/Director of Nursing. HCN offers a defined advancement path into administrative, clinical, nursing, quality, and compliance leadership roles. Compensation is $36–$40 per hour for part-time or PRN work, plus mileage, with overtime and quarterly bonus opportunities. Full-time RNCMs may choose hourly or salaried pay and may qualify for additional benefits, although this posting is for part-time or PRN employment.
