About The Position

Home Care Network, Inc. is seeking an experienced, self-motivated home health Registered Nurse Case Manager to provide RN case management and recertification services to clients across the far-western Northwestern Ohio region. This is a PRN role intended for a long-term working relationship; contractors are also considered. Initial work is expected to include 1–2 weekly medication setups and 1–10 recertification visits per week, with additional services and hours potentially added as needs arise. Growth into a part-time or full-time position may be possible but is not guaranteed. The role includes case management, starts of care and admissions, recertifications, transitional care and resumptions of care, clinical reviews, assessments, and other nursing services. It may also include new-hire orientations, in-service education, supervisory reviews, and input on training, care practices, and outcomes. The RN coordinates with physicians, agency staff, administrators, caregivers, clinicians, clients, and families, and manages or supervises RN, LPN, home health aide, and personal care aide staff. The position serves Defiance, Paulding, and Van Wert Counties and surrounding areas, with travel within a one-hour radius required. Willingness to travel up to two hours is highly desired. Visits are prioritized near the employee’s home where possible and are scheduled around availability and travel preferences. The typical schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m.; occasional early-evening and rare weekend visits may occur but are not required. No overnight shifts, overnight travel, on-call duties, or overtime shifts are required. State Survey support at the Perrysburg/Toledo office may be needed 1–2 times per year. The agency describes itself as nurse-owned and locally led, with a client-centered culture and quality, compliance, and training support.
