About The Position
Sage Group HealthCare Staffing is seeking a Home Health Registered Nurse for a travel/contract position in Mason, OH. This role involves providing and coordinating care for assigned Home Health patients, managing a caseload, and developing/implementing nursing care plans. The nurse will empower patients and families through education on disease management, provide care within the scope of practice, ensure physician orders are transcribed correctly, and continuously assess and document patient status, notifying physicians of significant changes. The position offers a guaranteed 40-hour work week, Monday through Friday, with productivity expectations. Paid orientation may last up to two weeks.
Requirements
- Registered Nurse: 2 years - Required
- Recent/Current Home Health Nursing experience: 1 year within the last 3 years – Required
- OASIS experience - Required
- Home Care Home Base (HCHB) charting experience – Required
- Must be Covid Vaccinated or provide medical or religious exemption
- ASN or an equivalent – Required
- RN License for the state applying or compact if applicable - Required
- BLS Certification from AHA - Required
Nice To Haves
- BSN or an equivalent – Highly preferred
Responsibilities
- Provide and coordinate care for assigned Home Health patients
- Manage assigned case load
- Develop and implement nursing care plans for patients as assigned
- Empower patients and families to reach healthcare goals by educating them on disease management
- Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented
- Notify physicians of significant changes in status
- Document and complete charting
