About The Position

Home Care Providers of Texas is hiring a full-time Registered Nurse to provide home health care in the Denton/North Dallas, Texas area. The nurse will deliver skilled, individualized care in patients’ homes and work with an interdisciplinary team to support patient outcomes.

The role uses Roger, an AI-driven, voice-enabled, HIPAA-compliant documentation platform intended to reduce charting time, and includes electronic charting through Kinnser (WellSky). Nurses receive 24/7 clinical and administrative support and paid training and onboarding. The posting describes a flexible home health role in which more than 90% of time is spent on patient care.

Requirements

• Active RN license in Texas or a Compact state.
• Active CPR certificate.
• Prior experience in geriatric home health care.
• Self-starter with organizational skills.
• Compassionate approach to care.
• Valid driver’s license.

Responsibilities

• Provide high-quality, comprehensive skilled nursing care in patients’ homes in accordance with physician orders and individualized plans of care.
• Partner with the interdisciplinary care team to develop, implement, and update personalized care plans.
• Administer nursing treatments and manage clinical activities to support optimal health outcomes.
• Continuously assess patient progress and the effectiveness of services.
• Communicate findings with the care team, patient, family, physician, and case manager.
• Perform additional duties as needed.
