About The Position

Elara Caring is seeking a compassionate Registered Nurse Home Health to provide care in Chicago and the western Chicago suburbs. The nurse assesses patients’ care needs, completes initial and ongoing assessments, and provides nursing care in accordance with the plan of care and physician orders. The role includes monitoring patients and reporting changes to the care team, educating patients and families about care and safety, responding appropriately to changes in patient condition, and helping ensure quality, safe, continuous care. The nurse must protect patient and staff privacy in accordance with HIPAA.

Requirements

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate’s, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.
