About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. In this role, the nurse guides patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care, provides education, coaching, and treatment-decision support, and collaborates with a Care Advocate and Social Worker. The position includes patient visits and certain non-visit activities, with compensation varying according to the work performed and productivity. The role also includes on-call responsibilities as assigned.

Requirements

Current RN license specific to the state(s) assigned for work.
Valid driver's license, reliable transportation, and liability insurance.
Current CPR certification.
At least 1 year of clinical experience as a Registered Nurse (RN).

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines based on the patient’s identified needs, or document the rationale for not making referrals.
Promote patient health and independence through teaching and appropriate rehabilitative measures, and assist patients in learning self-care techniques.
Supervise LPNs and HHAs.
Complete documentation timely and accurately.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
