About The Position
CenterWell Home Health is hiring a Registered Nurse to provide personalized, one-on-one skilled nursing care to patients in their homes. Reporting to the Clinical Manager, the nurse works with physicians and other clinicians to manage care plans that support patient recovery and independence. The role includes medication administration, wound care, patient assessments, condition monitoring, patient and family education, documentation, care coordination, and oversight of Home Health Aides. This is a patient-facing position covered by Humana’s TB screening program. It also participates in Humana’s driver safety program and requires a valid state driver’s license, personal vehicle liability insurance, and reliable transportation. The role is scheduled for 40 hours per week. The posting lists a pay-per-visit/unit rate of $49.00–$69.00 and a full-time annual base-pay range of $77,200–$106,200, with actual compensation varying based on location and job-related factors.
Requirements
- Med-surg, ICU, ER, or acute-care experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and families about disease management, medications, and treatment options.
- Maintain accurate patient-care records and coordinate with other healthcare professionals.
- Report patient-care and condition progress to the patient’s physician and the Clinical Manager.
- Oversee Home Health Aides and monitor their completion of documentation.
