About The Position

Home Care Providers of Texas is seeking compassionate, detail-oriented full-time registered nurses to provide home health care in the North Dallas area, with coverage in Denton. Nurses deliver care in patients’ homes and work with an interdisciplinary team to support individualized care plans and patient outcomes. The role uses the AI-driven, voice-enabled, HIPAA-compliant Roger app to streamline documentation and help nurses stay on top of OASIS requirements. Electronic charting is also provided through the Kinnser (WellSky) system. The position offers a remote workforce model, with more than 90% of time spent on patient care and 24/7 clinical and administrative support.

Requirements

• Active Texas or Compact RN license.
• Active CPR certificate.
• Prior experience in geriatric home health care.
• Self-starter with organizational skills.
• Compassionate approach to care.
• Valid driver’s license.

Responsibilities

• Provide comprehensive skilled nursing care in patients’ homes according to physician orders and individualized plans of care.
• Collaborate with the interdisciplinary care team to develop, implement, and update personalized care plans.
• Administer nursing treatments and manage clinical activities to support patient health outcomes.
• Assess patient progress and service effectiveness, and communicate findings to the care team, patient, family, physician, and case manager.
• Perform additional duties as needed.
