About The Position

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will deliver personalized care focused on functional improvement and helping patients overcome limitations and improve their quality of life. The Home Health Physical Therapist Assistant administers the physical therapy treatment plan developed by a Physical Therapist, provides therapeutic interventions, documents care, and coordinates with clinical staff. The role involves working in patients’ homes and applying infection-control practices during patient care.

Requirements

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population.
Ability to read, write, and speak fluent English.

One year of experience as a Physical Therapy Assistant.

Responsibilities

Administer the Physical Therapist-developed treatment plan to patients in their homes.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director about equipment failures, lack of supplies, or staff or client changes.
Apply infection-control concepts and universal precautions when performing patient care activities.
