About The Position

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will deliver personalized care focused on functional improvement and help patients overcome limitations and improve their quality of life. The role administers the physical therapy treatment plan developed by the Physical Therapist and includes patient care, documentation, coordination, and infection-control practices. This is a per-diem, patient-facing position involving work in patients’ homes.

Requirements

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
Ability to read, write, and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan developed by the Physical Therapist to patients in their homes.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities to patients.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director of equipment failures, lack of supplies, or staff or client changes.
Apply infection-control concepts and universal precautions while performing patient care activities.
