Home Health Physical Therapist Assistant-Per Diem
About The Position
As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will deliver personalized care focused on functional improvement, empowering individuals to overcome limitations and improve their quality of life. The role is per diem and patient-facing, and requires participation in Humana's TB screening program and driver safety program. The position has 1 scheduled weekly hour. The listed annual pay range is $64,000–$87,500 for full-time employment; actual pay varies based on location and job-related qualifications. Requirements
- Current state license to practice Physical Therapy.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
- Ability to read, write, and speak fluent English.

Responsibilities
- Administer the physical therapy treatment plan developed by the Physical Therapist to patients in their homes.
- Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities to patients.
- Document client records and submit clinical documentation to the office on time each week.
- Attend educational meetings and coordinate department in-services.
- Notify the Physical Therapist or Branch Director about equipment failures, lack of supplies, or staff or client changes.
- Apply infection-control concepts and universal precautions while performing patient care activities.
