About The Position

CenterWell Home Health is hiring a per-diem Home Health Physical Therapist Assistant who reports to the Clinical Manager. The role helps patients regain strength, mobility, and independence in their homes by delivering personalized care focused on functional improvement. The assistant administers the physical therapy treatment plan developed by a Physical Therapist, provides therapeutic exercises and modalities, documents care, coordinates with the care team, and follows infection-control and universal-precaution practices. This is a patient-facing role that requires TB screening and participation in Humana’s driver safety program.

Requirements

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population.
Fluent in reading, writing, and speaking English.

Responsibilities

Administer the physical therapy treatment plan developed by the Physical Therapist to patients in their homes.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director of equipment failures, lack of supplies, or changes involving staff or clients.
Apply infection-control concepts and universal precautions when performing patient-care activities.
