About The Position

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will deliver personalized care focused on functional improvement, administering a physical therapy treatment plan developed by a Physical Therapist. The role is per diem and patient-facing, with care delivered in patients’ homes. It requires participation in Humana’s TB screening program and driver safety program, including maintaining a valid state driver’s license and required personal vehicle liability insurance.

Requirements

Current state license to practice Physical Therapy.
Valid driver’s license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
Ability to read, write, and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan developed by the Physical Therapist to patients in their homes.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director of equipment failures, lack of supplies, or changes involving staff or clients.
Apply infection-control concepts and universal precautions while performing patient care activities.
