About The Position

Interim HealthCare of Oklahoma City is hiring full-time and part-time Home Health LPNs to provide skilled nursing visits in patients’ homes across the Oklahoma City metro area. LPNs work according to a plan of care established by an RN case manager, building relationships with patients while receiving support from a local team. Weekday visits are scheduled with the nurse’s input, and the agency aims to keep visits near the nurse’s home when patient needs allow. Staffing and patient needs may require visits outside the nurse’s preferred area, so flexibility across the metro is important. The service area includes Oklahoma City, Edmond, Norman, Moore, Yukon, Midwest City, Mustang, Guthrie, Choctaw, Wellston, Harrah, Piedmont, and Blanchard. Full-time nurses work two weekends per month and receive a weekday off following each weekend worked. Part-time nurses work one weekend per month. Pay is per visit: $30 for full-time and $22 for part-time. On-call pay is $40 per day Monday through Friday and $100 per day Saturday and Sunday. Mileage is reimbursed at $0.64 per mile, and employees are paid weekly on Fridays. Requirements

Active Oklahoma LPN license or multistate compact license.
At least 1 year of nursing experience.
Confidence working independently in the home setting.
Strong communication and documentation skills.
Valid driver’s license, reliable transportation, and current auto insurance.
Current CPR certification.
Flexibility to travel across the Oklahoma City metro as staffing and patient needs require.

Nice To Haves

Home health experience is a plus.

Responsibilities

Provide skilled nursing visits to home health patients according to the plan of care established by the RN case manager.
Perform assessments, take vital signs, provide wound care, and manage and educate patients about medications.
Teach patients and families about diagnoses, medications, and home safety.
Report changes in patients’ conditions to the RN case manager and physician in a timely manner.
Document each visit accurately and on time in the electronic medical record.
Coordinate with RNs, therapists, aides, and the office team to keep care on track.
