About The Position
Iowa Home Care is hiring a Licensed Practical Nurse (LPN) to deliver skilled, compassionate nursing care in clients’ homes in the Des Moines metro area. The LPN implements physician-directed care plans, administers treatments, and collaborates with an interdisciplinary team to support clients’ health, independence, and well-being. The role involves home visits and shifts and offers autonomy, teamwork, and the opportunity to build lasting relationships with clients.
Requirements
- Must be licensed as an LPN in Iowa.
- Knowledge of nursing standards established by the Iowa Board of Nursing.
- Strong communication skills.
- Commitment to confidentiality.
- Valid driver’s license and reliable transportation.
Nice To Haves
- One to two years of experience in acute hospital or home health care.
Responsibilities
- Deliver care according to the physician’s plan and established nursing standards.
- Conduct home visits and shifts, assess client conditions, record vital signs, and note changes.
- Administer nursing procedures, manage medications, and provide treatments according to physician orders and scope of practice.
- Promote a safe environment by educating families about home safety, infection control, and fall prevention.
- Communicate with the RN Case Manager and other team members to coordinate and improve care.
- Document care accurately and in a timely manner.
- Train families in home care practices and collaborate creatively to meet care objectives.
