About The Position

Iowa Home Care is hiring a Licensed Practical Nurse (LPN) to deliver skilled, compassionate nursing care in clients’ homes in the Des Moines metro area. The LPN implements physician-directed care plans, administers treatments, and works with an interdisciplinary team to support clients’ health, independence, and well-being. The role involves autonomous home visits and shifts while building relationships with clients and their families.

Requirements

• LPN licensed in Iowa.
• Strong communication skills and commitment to confidentiality.
• Valid driver’s license and reliable transportation.

Nice To Haves

• 1–2 years of experience in acute hospital or home health care.
• Knowledge of nursing standards set by the Iowa Board of Nursing.

Responsibilities

• Deliver care according to the physician’s plan and established nursing standards.
• Conduct home visits and shifts, assess client conditions, record vital signs, and note changes.
• Administer nursing procedures, manage medications, and provide treatments in accordance with physician orders and scope of practice.
• Promote a safe environment by educating families about home safety, infection control, and fall prevention.
• Communicate with the RN Case Manager and other team members to coordinate care and improve care quality.
• Document all care accurately and in a timely manner.
• Train families in home care practices and collaborate creatively to meet care objectives.
