Home Health Licensed Practical Nurse - PRN
About The Position
As a CenterWell Home Health Licensed Practical Nurse, you provide personalized, one-on-one care to help patients regain independence in their homes. Reporting to the Clinical Manager, you work with physicians and clinicians to manage care plans that support patients’ recovery. The role provides skilled nursing care in patients’ homes under the direction of an RN and includes patient and family education, care coordination, and discharge planning. This is a PRN role with 1 scheduled weekly hour. The role is patient-facing and requires TB screening. It also participates in Humana’s driver safety program, requiring a valid state driver’s license and personal vehicle liability insurance at the applicable required limits.
Requirements
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
- Must undergo TB screening if selected.
- Must maintain personal vehicle liability insurance at the applicable state minimum limits or the specified limits, whichever is higher.
Nice To Haves
- Nursing experience in a Home Health or Hospice setting.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Communicate promptly with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
