About The Position
Interim HealthCare is seeking a Home Health Licensed Practical Nurse (LPN) to provide exceptional patient care in patients’ homes. The role offers flexibility and autonomy: nurses can set a schedule that works for them and choose how much they work. Care is provided under the supervision of an RN, often to patients recovering after hospital discharge or living with illnesses, injuries, and chronic diseases. The LPN works with a team to deliver quality care, contributes to patients’ plans of care, and communicates with the RN supervisor and interdisciplinary team. The role includes a 1:1 nurse-to-patient ratio. Interim HealthCare is a home care company founded in 1966 and operating through more than 300 offices. Requirements
- Graduate of an accredited practical nursing program.
- Active California LPN license.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing, strong communication skills, and compassion.

- At least one year of experience is preferred.
Responsibilities
- Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after hospital discharge, under RN supervision.
- Work with a team to deliver quality patient care.
- Take patient vital signs, perform assessments, document progress, and report changes to the RN supervisor.
- Assist the RN in carrying out the patient’s plan of care and teach patients self-care techniques.
- Educate families about the patient’s plan of care, medications, and home safety.
- Collaborate with the interdisciplinary team to coordinate patient care.
