About The Position

Elite Care Management is hiring a Licensed Practical Nurse to provide one-on-one home health care to a patient for the duration of each shift. The nurse gets to know the patient and family, learns about care needs in the home, and plans, organizes, and assists with home health care services. The role supports patients’ daily living activities, comfort, overall well-being, and quality of life. The position is offered full-time and part-time, with shifts listed as 6 a.m.–6 p.m. Sunday and 6 a.m.–6 p.m. PRN on various weekdays and weekends. The pay range is $35–$40 per hour. A sign-on bonus of $5,000 for full-time or $3,000 for part-time is advertised.

Requirements

Current RN or LPN state license.
Current CPR card.
Driver’s license.
Social Security card.
Auto insurance card.
TB test results: a two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last five years.

Responsibilities

Observe patients, record significant conditions and reactions, and notify a supervisor or physician about patient conditions, reactions to drugs or treatments, and significant incidents.
Follow organizational and nursing policies and procedures.
Communicate effectively, both verbally and in writing.
Manage and direct patient care according to the patient care plan, updating the plan as needed.
Administer prescribed medications and treatments and monitor and document their effectiveness.
Respond as appropriate to changes in a patient’s condition and report those changes to the appropriate physician or supervisor.
Assist patients with daily living activities, comfort, and overall well-being.
