About The Position
As a CenterWell Home Health nurse, the Licensed Practical Nurse reports to the Clinical Manager and provides personalized, one-on-one care to help patients regain independence in their homes. The nurse works with physicians and clinicians to manage care plans that support recovery. The role provides skilled nursing care in patients’ homes under the direction of an RN, implements individualized care plans in collaboration with patients, families, and the healthcare team, and supports patient education and care coordination.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
Nice To Haves
- Nursing experience in a Home Health or Hospice setting.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families about disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Communicate promptly with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
