Home Health Licensed Practical Nurse-Per Diem
About The Position
As a CenterWell Home Health nurse, the Home Health Licensed Practical Nurse reports to the Clinical Manager and provides personalized, one-on-one care to help patients regain independence in their homes. The nurse works with physicians and clinicians to manage care plans supporting patient recovery. This role provides skilled nursing care in patients’ homes under the direction of a registered nurse and collaborates with patients, families, and the healthcare team to implement individualized care plans. The position is per diem, with one scheduled weekly hour listed. Compensation is described as a pay-per-visit/unit rate of $40.00–$57.00. The posting also lists an estimated annual base-pay range of $63,600–$87,600 for full-time, 40-hour-per-week employment, which may vary by geography and job-related qualifications. The role is patient-facing and requires TB screening. It is also subject to Humana’s driver safety program and requires a valid driver’s license and personal vehicle liability insurance meeting applicable limits.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families about disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
