Home Health Licensed Practical Nurse-Per Diem
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, the Licensed Practical Nurse provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. The role works with physicians, clinicians, patients, and families to manage individualized care plans that support recovery and help patients regain independence. The position is per diem and paid at a per-visit/unit rate. It is patient-facing and requires participation in Humana’s tuberculosis screening program. The role also participates in Humana’s driver safety program and requires a valid state driver’s license and personal vehicle liability insurance meeting applicable requirements.
Requirements
- Current nursing license in the state where the employee practices.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
- Participation in required tuberculosis screening.
- Maintain personal vehicle liability insurance in accordance with the applicable state minimums or the stated limits, whichever is higher.
Nice To Haves
- Nursing experience in a Home Health or Hospice setting.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with patients, families, and the healthcare team.
- Educate patients and families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Promptly communicate patient or family needs and changes in status to patients, physicians, clinical managers, and other healthcare team members.
- Participate in care coordination activities and discharge planning.
