About The Position
As a CenterWell Home Health nurse, the Home Health Licensed Practical Nurse provides personalized, one-on-one care to help patients regain independence in their homes. Reporting to the Clinical Manager and working under the direction of an RN, the nurse works with physicians, clinicians, patients, and families to manage care plans that support recovery. The position involves home visits and care coordination, including discharge planning. This is a per-diem role with one scheduled weekly hour. Pay is listed at a per-visit/unit rate of $33.00–$47.00. The posting also gives a full-time annual base-pay estimate of $53,000–$73,000. The role is patient-facing and requires TB screening. It is included in Humana's driver safety program, with requirements for a valid state driver's license and personal vehicle liability insurance.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families about disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
