About The Position

As a Home Health Intake Coordinator with Home Health Care, Inc., you'll be one of the first people responsible for turning a new referral into an organized plan of care. You'll help move patients from “referred” to “ready for care.” Great intake coordinators know how to manage a lot of moving pieces without losing sight of the patient behind them. You'll communicate with hospitals, physicians, referral partners, clinicians, patients, and families to make sure we have what we need to begin care. Intake isn't a position where everything arrives neatly one task at a time. Priorities change. A new referral comes in. A hospital needs an answer. Documentation is missing. A schedule needs to be adjusted. A family calls with a question. Then another referral hits your desk. If you're someone who can prioritize quickly, communicate clearly, stay organized, and keep things moving, those skills are incredibly valuable here.

Requirements

  • Previous healthcare intake, admissions, referral coordination, or scheduling experience
  • Home health experience strongly preferred
  • Understanding of medical and insurance terminology
  • Experience verifying payer or reimbursement information
  • Strong EMR and computer skills
  • Excellent written and verbal communication
  • Ability to confidently communicate with referral sources, patients, families, and clinicians
  • Strong attention to detail
  • Excellent organization and prioritization
  • The ability to stay composed when several things need attention at once
  • A positive, team-oriented attitude

Responsibilities

  • Reviewing incoming home health referrals
  • Determining whether referrals meet admission criteria
  • Verifying payer and reimbursement eligibility
  • Tracking down missing orders or medical documentation
  • Communicating with referral sources
  • Speaking with patients and families during intake
  • Coordinating the appropriate nursing, therapy, or aide services
  • Scheduling visits according to discipline and frequency requirements
  • Entering accurate information into the EMR
  • Helping resolve barriers that could delay the start of care
