About The Position

The Home Health Field LVN/LPN delivers nursing care to patients in their homes, helping ensure comfort, safety, and effective management of health conditions. The role includes assessing patient health status, administering medications, providing wound care, and collaborating with physicians, therapists, family members, and other healthcare professionals. The LVN participates in developing and implementing individualized care plans under the direction of an RN and in accordance with the plan of care. The position also involves educating patients and caregivers about treatment protocols and health maintenance to support independence, recovery, and chronic disease management.

Requirements

• Current LVN license.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

Responsibilities

• Under the direction of the RN, assist in identifying patients’ physical, psychosocial, and environmental needs, as evidenced by documentation, the clinical record, team reports, and evaluations.
• Participate in planning and implementing care with the RN in accordance with the plan of care.
• Apply infection-control concepts and standard precautions when coordinating and performing patient-care activities to prevent contamination and disease transmission.
• Assure continuity of care through delivery of quality patient care.
• Communicate effectively with patients and families, team members, and other healthcare professionals, as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with third-party payor requirements.
• Promote agency philosophy and administrative policies to ensure quality of care.
• Assess patient health status, administer medications, provide wound care, and educate patients and caregivers on treatment protocols and health maintenance.
