About The Position

The Home Health Field LVN/LPN delivers nursing care to patients in their homes, supporting comfort, safety, health-condition management, recovery, and chronic disease management. The role includes assessing patient health status, administering medications, providing wound care, and collaborating with physicians, therapists, family members, and other care-team professionals. The LVN helps develop and implement individualized care plans that promote patient independence and well-being, and educates patients and caregivers on treatment protocols and health maintenance. Under the direction of an RN, the LVN assists in identifying patient needs and participates in planning and providing care in accordance with the plan of care.

Requirements

• Current LVN license.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

Responsibilities

• Under the direction of the RN, assist in identifying patients’ physical, psychosocial, and environmental needs, using documentation, clinical records, team reports, and evaluations.
• Participate in planning and implementing care with the RN in accordance with the plan of care.
• Apply infection-control concepts and standard precautions when coordinating and performing patient-care activities to prevent contamination and disease transmission.
• Assure continuity of care through delivery of quality patient care.
• Communicate effectively with patients and families, team members, and other healthcare professionals, as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with third-party-payer requirements.
• Promote the Agency’s philosophy and administrative policies to support quality of care.
• Assess patient health status, administer medications, provide wound care, and educate patients and caregivers on treatment protocols and health maintenance.
