About The Position

The Home Health Field LVN/LPN delivers nursing care to patients in their homes, supporting comfort, safety, and management of health conditions. The role includes assessing patient health status, administering medications, providing wound care, and collaborating with physicians, therapists, family members, and other care team members. The LVN helps develop and implement individualized care plans that promote patient independence and well-being. The position also involves educating patients and caregivers about treatment protocols and health maintenance, supporting recovery and chronic disease management in a home setting.

Under the direction of an RN, the Home Health LVN assists in identifying patient needs and participates in planning and implementing care according to the plan of care. The LVN applies infection-control practices, promotes continuity and quality of care, communicates effectively with patients, families, and care professionals, monitors assigned cases for third-party payer compliance, and supports the agency’s philosophy and administrative policies.

Requirements

• Current LVN license.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

Responsibilities

• Under the direction of the RN, assist in identifying patients’ physical, psychosocial, and environmental needs based on documentation, clinical records, team reports, and evaluations.
• Participate in planning and implementing care in conjunction with the RN and in accordance with the plan of care.
• Apply infection-control concepts and standard precautions when coordinating and performing patient care activities.
• Assure continuity of care through delivery of quality patient care.
• Communicate effectively with patients and families, team members, and other health care professionals, as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Promote agency philosophy and administrative policies to ensure quality of care.
• Assess patient health status, administer medications, provide wound care, and educate patients and caregivers on treatment protocols and health maintenance.
