About The Position

The Home Health Field LVN/LPN delivers nursing care to patients in their homes, supporting comfort, safety, recovery, and chronic disease management. The role assesses patient health status, administers medications, provides wound care, and collaborates with physicians, therapists, family members, and other care team members. Working under the direction of an RN, the LVN assists with identifying patient needs and participates in planning and implementing individualized care in accordance with the plan of care. The position also educates patients and caregivers about treatment protocols and health maintenance, with the goal of promoting patient independence and improving well-being.

Requirements

• Current LVN license.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

Responsibilities

• Under the direction of the RN, assist in identifying patients’ physical, psychosocial, and environmental needs using documentation, clinical records, team reports, and evaluations.
• Participate in planning and implementing care in conjunction with the RN and in accordance with the plan of care.
• Apply infection-control concepts and standard precautions while coordinating and performing patient-care activities to prevent contamination and disease transmission.
• Assure continuity of care through delivery of quality patient care.
• Communicate effectively with patients and families, team members, and other health care professionals, as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with third-party payor requirements.
• Promote the Agency’s philosophy and administrative policies to ensure quality of care.
• Assess patient health status, administer medications, and provide wound care.
• Educate patients and caregivers on treatment protocols and health maintenance.
