About The Position

The Home Health Field LVN/LPN delivers nursing care to patients in their homes, supporting comfort, safety, health-condition management, recovery, and chronic disease management. The role assesses patient health status, administers medications, provides wound care, and works with physicians, therapists, family members, and caregivers. Under the direction of an RN, the LVN assists in identifying patient needs and participates in planning and implementing care according to the plan of care. The position also helps educate patients and caregivers on treatment protocols and health maintenance to promote independence and well-being.

Requirements

• Current LVN license.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

Responsibilities

• Under the direction of the RN, assist in identifying patients’ physical, psychosocial, and environmental needs using documentation, clinical records, team reports, and evaluations.
• Participate in planning and implementing care with the RN in accordance with the plan of care.
• Apply infection-control concepts and standard precautions when coordinating and performing patient care activities.
• Maintain continuity of care through the delivery of quality patient care.
• Communicate effectively with patients and families, team members, and other healthcare professionals, as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases for compliance with third-party payer requirements.
• Promote the Agency’s philosophy and administrative policies to support quality of care.
• Assess patient health status, administer medications, and provide wound care.
• Educate patients and caregivers about treatment protocols and health maintenance.
